For Visiting Physicians

Thank you for your interest in taking a rotation at Rush University Medical Center. Our residency and fellowship programs welcome visiting physicians to participate in rotations if space is available. Please contact Jennifer Hayes to determine if your request and timing can be accommodated.

Licensing requirements

All visiting physicians must be currently in an Accreditation Council for Graduate Medical Education-approved training program in their home institution and be licensed to practice in Illinois.

Please provide the following:

  • Proof of your ACGME-approved residency/fellowship training program
  • A copy of your permanent (#036) Illinois license or a copy of your temporary Illinois training license (#125)

If you are not licensed in Illinois or are licensed in another state, you must apply for an Illinois license no matter how short your rotation. Two options are available:

  1. A temporary limited license (code # 130) covers the exact dates of the rotation only. For more information, please contact Jennifer Hayes at (312) 942-2729.
  2. A permanent license (code # 036) is valid for up to three years (depending on issuance date). This is the best option if you plan to move to/stay in/return to Illinois often. Check the description to ensure that you qualify.

Application process

  1. After receiving approval from Rush GME, please download and print the visiting resident rotation form and clearly print or type all information requested (except the two boxes labeled “internal use only”). For those already holding a valid Illinois license, the completed form and all required documents must be received by the Office of Graduate Medical Education at least 60 days prior to the rotation start date. For those not currently licensed in Illinois, allow at least 90 days to include the Illinois licensure process.
  2. Visit the Illinois Department of Financial and Professional Regulation website if you need to apply for an IL medical license.
  3. Please submit your one page Rush visiting resident rotation form plus all additional required materials as stated on the form:
  • A copy of your Illinois medical license (if you have one)
  • Curriculum vitae or previous Electronic Residency Application Service application
  • Letter of permission from your home institution/program director
  • Verification of malpractice insurance coverage from your home institution
  • Copy of a valid Educational Commission for Foreign Medical Graduates certificate (if applicable)
  • Proof of completion of a Health Insurance Portability and Accountability Act, or HIPAA, program at your home institution
  • Copy of your immunization records or health clearance form from your employee health office
  • Signed Program Letter of Agreement (this will be provided to you by Rush GME)
  • Signed Privilege matrix (this will be provided to you by Rush GME)

Other requirements

  • Please confirm orientation and first-day meeting times and locations with your program contact person.
  • Complete the required four-hour Epic training class at Rush before your rotation begins. (Jennifer Hayes will help you enroll.)
  • Obtain an ID badge from Rush security, Tower Building, Lower Level 1502. (Secure necessary paperwork from Jennifer.)
  • Parking at Rush
  • Your ID badge is required for discounted parking in the Rush parking garage at $40 per month.
  • A $30 refundable deposit must be paid by cash or check on the first day of your rotation.
  • On your first day, enter the visitor’s parking entrance A or B and bring your receipt to the parking office. After receiving your AVIT transponder, use employee parking entrances C through F. When your rotation ends, return the AVIT transponder for a full refund of your deposit.
