About Rush University

Dear Incoming House Officers,

Welcome to Rush University Medical Center for the upcoming academic year. As part of your condition of employment, both at Rush and for rotations to affiliated hospitals, you are required by federal law to meet certain infection control requirements.

Before your start date, you must provide: a 2-Step Mantoux tuberculosis screening and laboratory titers documenting your immunity to measles (if born after 1956), mumps, rubella, chicken pox, hepatitis B. In reference to tuberculosis screening, please provide all documentation of testing done within the past year. The last test must be done after April 1st of the current year. Will also accept results of a Quantiferon TB blood test done within 6 months of start date. Please inform the Employee Health staff if you have had a positive TB test in the past.

For your convenience, and, in order to avoid delays in the start of your employment, please print out our health form and have it filled out by your primary care provider or student/employee health services.

Return the health form by mail or fax along with copies of your TB screening and serology results to:

Rush University Medical Center
Employee & Corporate Health Services (ECHS)
1653 West Congress Parkway
Suite 100 Senn
Chicago, IL 60612
Fax (312) 942-8021

In most cases, you already have had these tests, as all health care institutions are under similar regulations. In addition, all new incoming house officers must report to ECHS for the mandatory urine drug screen and fit test for TB Mask prior to start date.

If you have any questions or need assistance in meeting these requirements, please contact Employee & Corporate Health Services at (312) 942-5878. Office hours are 7:30 a.m. to 4:00 p.m., Monday through Friday.

Thank you for your cooperation. We are looking forward to working with you.

Best wishes in your endeavors.

Sincerely,

Reneta Chwalisz, MBA, RN
