The Neurocritical Care fellowship is a two year program that provides advanced clinical and research training in Rush University Neurosciences Intensive Care Unit, providing 24-hour primary team coverage fulfilling UCNS training requirements.
Specific skills to be mastered include airway and ventilator management, traditional and ultrasound guided central venous line placement, pulmonary artery catheterization, arterial line placement, transcranial dopplers, echocardiography, bronchoscopy, and continuous EEG monitoring. Typical admissions to the NSICU include subarachnoid hemorrhage, intracerebral hemorrhage, malignant ischemic strokes, traumatic brain injury, status epilepticus, meningitis/encephalitis, myasthenic crisis, and Guillan-Barre syndrome. In addition to time spent in the neurocritical care unit, fellows will complete rotations in the medical intensive care unit, the coronary care unit, the trauma unit, and in neurosurgical anesthesiology. Elective time will be provided for fellows to participate in other rotations such as endovascular neurology/neurosurgery or research. Fellows will also gain knowledge with implementation and management of hypothermia after cardiac arrest.
During their training, fellows will be strongly encouraged to participate in research with goals of presenting at national meetings and publishing in journals. A monthly journal club and weekly research meeting will provide a foundation to learn basic research principles. Fellows will be provided with a discretionary fund to cover the costs for educational and research endeavors. Our division is currently involved in several NIH and foundation funded studies that fellows will have the opportunity to be involved with during their training. Independent investigative work will be fostered with close mentorship and faculty support. Workshops on grantsmanship and IRB applications are also available for those interested.
Fellows are allotted four weeks vacation time and work hours are governed by guidelines set forth by the UCNS. Salary is commensurate with PGY level.
The Neurosciences Intensive Care Unit at Rush is a state-of-the-art specialty unit of 25 beds with advanced neuro-monitoring and neuro-therapeutic capabilities including continuous EEG, Licox brain tissue oxygen sensor, cerebral blood flow devices, and cooling devices. On-site CT scanning is available within the ICU capable of performing CT angiography and perfusion. The NSICU is staffed full time by 5 neurointensivists, fellows, and residents from neurology, anesthesiology, and neurosurgery. Our team?also includes 5 dedicated neurocritical care nurse practitioners that manage patients within the unit. The NSICU encompasses a closed unit model with a dedicated ICU team providing primary care to patients. Bedside teaching rounds are conducted daily with pharmacy and nutrition in attendance. In January 2012, we will be expanding our unit further when we move to our new state-of-the-art Acute and Critical Care Tower.
