Please read the following information carefully so as to ensure our ability to honor your request.

1.	Use of the Transcript Request form is encouraged. If not used, the following information must be included in the request:   Indicate Number of Transcripts Needed   Indicate the exact address of where each transcript is to be sent. Four or more transcripts to different addresses require that preprinted address labels be provided with the request. Transcripts cannot be faxed.   Social Security Number   Date of Birth   Current Address of Student   Daytime Telephone Number   Signature of Student   Date of Request
2.	Transcripts are released only with prior written consent of the student.
3.	Letter or fax requests are accepted. Please remember to include your signature. E-mail requests cannot be accepted.
4.	Please allow 3-4 working days for request to be processed.

Transcripts will not be released if the student has an outstanding financial obligation to the University.
