Rush faculty, staff, employees, fellows and students engaged in research activities are required to register all research projects with the Office of Research Affairs, whether or not the project involves human or animal subjects and whether or not the project involves external funding support.
Investigators may also be required to file applications with the following Rush research committees:
Institutional Animal Care and Use Committee if any aspect of the project involves living or dead animal subjects, or tissue or body fluid from animals (submit to Comparative Research Center).
Institutional Biosafety Committee if any aspect of the project involves recombinant DNA, transgenic animals, pathogenic agents (whether human or animal), or biological toxins, download these forms found on the previous page:

Radiation Safety Committee if any aspect of the project involves radioactive material and/or radiation other than isotopes (submit directly to the Radiation Safety Office).
Deceased Subjects if any aspect of the project involves cadaverous materials, there are seven questions that need to be addressed; they can be found on page 2 of the Grants and Contracts Routing Form and will be reviewed at the time of grant submission.

This form is mandatory for ALL Grants and Contracts as of
November 1, 2001.
Applications must be submitted to the appropriate committee prior to submitting the grant application to Office of Sponsored Research Projects for review and signature of the grant application. Completed applications must include approval signature from the department chair or section director.
When all of the above steps have been completed, submit the Grants and Contract Routing Form, the sponsor application forms and budget to the Office of Sponsored Projects, located in room 529 of the Academic Facility.
