Financial Aid Forms For Summer 2012 - Spring 2013

Your financial aid application must be renewed each and every academic year. The financial aid year runs from summer term through the following spring term. Students are encouraged to apply by our priority deadline of May 1, 2012 but applications after that date will be honored.

APPLICATION DOCUMENTS

Required & Program-specific Forms
FAFSA	Required from all aid applicants.
Institutional Aid Application	Required from all aid applicants.
Parent Information Form	Required from all dependent students. Suggested for all independent students (including Graduate/Professional students) who have not submitted this information in the past.
Advanced Nurses Traineeship	Recommended for non-GEM graduate-level nursing students. Assistance is not guaranteed and is subject to funding.
Projected Nurse Enrollment	Required from non-GEM graduate-level nursing students.
Credit Policy Statement	Required from all incoming M1 medical students.
Nurse Faculty Loan Program (NFLP) Application	For non-GEM Nursing students interested in full-time faculty positions. More information about this program is available here.
Consortium Agreement Contract	PhD Students in Nursing may wish to use federal loans to pay for cognates at other institutions. Please note the deadline for submission on this form.
Financial Aid Transcript Request	College of Nursing students who had a Federal Nursing Loan at a prior institution.
Supplemental Forms (submit only if asked to)
Verification Worksheet (Dependent)	Dependent Undergrads selected for verification.
Verification Worksheet (Independent)	Independent Undergrads or Graduate/Professional students selected for verification.
Parent Asset Worksheet
Student Asset Worksheet
Business/Farm Supplement	Required for business or farm owners reporting a value for a family business or farm on the FAFSA.
