Financial Aid Forms For Summer 2009 - Spring 2010

| FAFSA | Required from all aid applicants. |
| Institutional Aid Application | Required from all aid applicants. Site registration information is removed annually, so you'll need to register as a new user to complete your form. |
| Parent Information Form |
Required from all dependent students. Suggested for all independent students (including Graduate/Professional students) who have not submitted this information in the past. |
| Advanced Nurses Traineeship | Recommended for non-GEM graduate-level nursing students who will be (1) enrolled in at least 9 credits per quarter or (2) part-time and in their final 12 months of study. Assistance is not guaranteed and is subject to funding. |
| Projected Nurse Enrollment | Required from non-GEM graduate-level nursing students. |
| Credit Policy Statement | Required from all incoming medical students. |
| Financial Aid Transcript Request | College of Nursing students who had a Federal Nursing Loan at a prior institution. |

| Lender Selection Form | Required for all Stafford/Grad PLUS borrowers each year. |
| Stafford Entrance Counseling | Required for all first-time Stafford borrowers at Rush. Click "Stafford entrance" link. |
| Stafford Application/Master Promissory Note | Required for all first-time Stafford borrowers at Rush, or for students switching lenders. Choose your lender, and complete this document on that lender's website. |
| Grad PLUS Entrance Counseling | Required for all first-time Grad PLUS borrowers at Rush. Click "Grad PLUS entrance" link. |
| Grad PLUS Application/Master Promissory Note | Required for all first-time Grad PLUS borrowers at Rush, or for students switching lenders. Choose your lender, and complete this document on that lender's website. Recommended to use same lender as Stafford loans for ease of repayment. |
